Property Package Program Rules and Regs
 
Persons In Our (Wisconsin Correction Institution) Care: Orders may be placed directly with Union Supply Direct where institutional policy allows. Please use provided order forms. See your property room staff for an order form.
 
FAMILY & FRIENDS: Family & Friends may place orders for residents online, by phone, fax or mail. Please check with your PIOC to ensure the PIOC is eligible to receive the specific items you are ordering. For more information please visit our website at WIpackageprogram.com.
 
PRODUCT: All items must have the item number, quantity, description, size and price marked clearly on your order form. Union Supply Direct attempts to show and describe merchandise and all related information as accurately as possible. However, Union Supply Direct does not warrant that product descriptions or other content of our catalog, order form or website is accurate, complete, reliable, current, or error-free. In addition, manufacturers may make changes in product design, color, package size and style at any time. Union Supply Direct reserves the right to update or correct item information and to change prices and offers at any time. If a product offered by Union Supply Direct is not as described, your remedy is to immediately return it, unused, upon receipt while complying with your facility’s rules and regulations. In the event USG is notified of a stock issue, manufacturer delay, or discontinuation of an item, USG will immediately inform WI DOC and provide an estimated timeframe they expect the item back in stock, or suggest another method of resolution as appropriate. Such issues are communicated to Friends & Family through the USG website and/or email blasts, and to PIOC via the IVR phone system and/or posted flyers.
 
ELIGIBILITY: PIOC’s are responsible for researching their eligibility for items as provided in DAI Policy 309.20.03 and are responsible for any returns that are non-complaint with their status.
 
PAYMENT METHODS: Union Supply Direct accepts the following forms of payment: MasterCard, Visa, Discover, money order (we only accept domestic money orders printed in the U.S.), cashier’s check, institution check, personal check (must have name, address, and phone number imprinted on check) and Paypal. Full payment must be made prior to an order being processed. We accept Credit/Debit cards and prepaid cards with Visa/MasterCard logos. There will be a hold on your credit card/debit card for the amount of the order at the time the order is placed. The hold on your card will be removed and your card will be charged at the time the package ships. For prepaid card payments, funds are deducted from your card and placed on hold at the time the order is placed. A positive balance must be kept in the account until the package ships at which time your card will be charged. For customers not located in Wisconsin, we accept Cash payments via ACE Cash Express, there are 900 ACE Cash Express locations nationwide, pleae check our website for more information.
 
SALES TAX: Sales tax is required by the State of Wisconsin. State and local taxes vary by the location of each facility. Please see the tax rate table on the next page for your specific tax rate. Actual sales tax may vary from the rate shown, and final tax rates are determined by the Wisconsin Department of Revenue. All products must come back to us directly from the facility. For security purposes, we cannot accept any product that has left the facility. You must return these goods within 30 days of receipt. All original boxes and packing material are required on any returned goods. These goods will be repaired or replaced at no cost to you. All merchandise that was ordered in error, for any reason, can be exchanged for $10 or 50% of the purchase price, whichever is lesser. All exchanges must be done within 30 days of purchase. Union Supply Direct is not responsible for refused items. Please ensure that the items you order are approved at your facility. If the original payment was made by credit or debit card, approved refunds will be credited back to the card used in the original purchase, including prepaid credit and debit cards. If a package or item(s) is returned by you or your facility for any reason other than because the item(s) are damaged or the wrong item(s) were sent, a restocking fee of $10 or 50% of the purchase price, whichever is lesser will be deducted from your refund. If the cost of the refused package or item(s) is less than the restocking fee, no refund will be issued. Shipping Fees are non-refundable for refused packages.

Hobby Package Program Rules and Regs
 
Hobby privileges are subject to change without notice. Persons In Our Care (PIOC) can receive a total of $100 of Hobby product each month, excluding tax. Musical Instruments are not included in the $100 limit and must be purchased separately. Limit resets on the 1st of each month.

ClearTunes Television Warranty Service Procedures
Union Supply Direct offers 180 day limited warranty service on all ClearTunes televisions. See the limited warranty enclosed with your television for the full terms of your warranty. To return a defective ClearTunes television within the 180 day warranty period, contact Union Supply Direct Customer Service Department for a prepaid UPS call tag by phone 866-404-8989 or mail PO Box 619059, Dallas, TX 75261-9059. Return the defective ClearTunes television unit to USG Repair Center 3321 Toy Rd. Groveport, OH 43125. The return package must include: (1) the defective television; (2) a copy of the original receipt, (3) all of the accessories that came with the product, and (4) your full name, PIOC number, facility name and physical address (no PO Boxes are accepted). Out of Warranty Service (181–365 days from original date of purchase): To return a defective ClearTunes television within the out of warranty period, send the unit to USG Repair Center 3321 Toy Rd. Groveport, OH 43125. The return package must include (1), (2), (3) and (4) above and a check payable to Union Supply Direct, in the amount of $90.00 for return shipping. If returned merchandise is found by Union Supply Direct or ClearTunes, in their sole discretion, to not be defective or you do not meet all applicable requirements above, then your returned product will not be processed and the product may be returned to you. See the limited warranty enclosed in your product packaging for the full terms of the warranty applicable to your ClearTunes television.
 
 
   
Quarterly Personal Care Package Program
  
ORDERING PERIOD: Family and Friends of Person’s in our cares at Wisconsin Correctional Institutions. Ordering period for this catalog begins May 1st – October 31st, 2024. Only Family and Friends may order starting November 1st.

ORDER LIMITS: Person’s in our cares may receive one (1) hygiene order per quarter. The order must meet a minimum of $20 before taxes or order will be refunded. The order must not exceed $75 before taxes. If the order exceeds the maximum limit of $75, Union Supply reserves the right to cancel and refund any items to meet the limit of $75. Person’s in our cares may only receive two (2) total of each item items as outlined in the catalog.

It is the person’s in our care’s responsibility to ensure that they comply with the limits and/or restrictions for inmate personal property as referenced in the DAI Policy 309.20.03 Inmate Personal Property and Clothing, Section 1, B, 1-15. Canteen, hygiene and property items are all considered personal property and count towards the 32”x 16” x16” cubic inch requirement outlined in the above policy.
 
Eligibility: General Population person’s in our cares are eligible to receive a package, General Population is identified in the WICS system as status “GENP”. Person’s in our cares located at DCI in the following housing units are also eligible 11FF, 25EW, 25SW, and 25WW.
 
Administrative Confinement person’s in our cares are eligible to receive a package. Administrative Confinement is identified in WICS system as status “AC”. Individuals in AC status will ONLY be allowed property in their cells consistent with DAI Policy and Facility Procedure. Items that are not allowed will be stored in their personal property until release.
 
Protective Confinement person’s in our care are eligible to receive package. Protective Confinement is identified in WICS as “PC”. Individuals in PC status will ONLY be allowed property in their cells consistent with DAI Policy and Facility Procedure. PRODUCT: All items must have the item number, quantity, description, size and price marked clearly on your order form. Union Supply Direct attempts to show and describe merchandise and all related information as accurately as possible. However, Union Supply Direct does not warrant that product descriptions or other content of our catalog, order form or website is accurate, complete, reliable, current, or error-free. In addition, manufacturers may make changes in product design, color, package size and style at any time. Union Supply Direct reserves the right to update or correct item information and to change prices and offers at any time. If a product offered by Union Supply Direct is not as described, your remedy is to immediately return it, unused, upon receipt while complying with your facility’s rules and regulations.
 
PAYMENT METHODS: Union Supply Direct accepts the following forms of payment: MasterCard, Visa, Discover, money order (we only accept domestic money orders printed in the U.S.), cashier’s check, or personal check (must have name, address, and phone number imprinted on check). Full payment must be made prior to an order being processed. We accept Credit/Debit cards and prepaid cards with Visa/MasterCard logos. There will be a hold on your credit card/debit card for the amount of the order at the time the order is placed. The hold on your card will be removed and your card will be charged at the time the package ships. For prepaid card payments, funds are deducted from your card and placed on hold at the time the order is placed. A positive balance must be kept in the account until the package ships at which time your card will be charged. The following forms of payments are not accepted: Green Dot, Cash App, Apple Pay.
 
SALES TAX: Sales tax is required by the State of Wisconsin. State and local taxes vary by the location of each facility. Please see the tax rate table on the order form for your specific tax rate. Actual sales tax may vary from the rate shown, and final tax rates are determined by the Wisconsin Department of Revenue. Please visit our website for the current tax rate which will be applied to your order. Sales Tax rates are subject to change at any time.

RETURNS: The person’s in our care will be given an opportunity to inventory the contents of the order in the presence of an employee from the facility. Please check all of your items and report any discrepancies at the time of order receipt. If any of these items are found to be defective or damaged in shipping, please notify the facility employee inventorying your order immediately. The facility employee will verify any discrepancy and report all discrepancies to Union Supply Direct. All verified discrepancies will be corrected by Union Supply Direct with replacement of product or reimbursement of funds. Union Supply is not responsible for any items ordered in error. Used or abused items cannot be returned, exchanged or refunded. All merchandise that was ordered in error, for any reason, can be exchanged for $6.95. Returns and exchanges must be made within 30 days of delivery. Any requests made after this date will not be honored. If the original payment was made by credit or debit card, approved refunds will be credited back to the card used in the original purchase, including prepaid credit and debit cards.
 
RESTOCKING FEES: If a package or item(s) is returned by you or your facility for any reason other than because the item(s) are damaged or the wrong item(s) were sent, a restocking fee of $5.00 will be deducted from your refund. If the cost of the refused package or item(s) is less than the restocking fee, no refund will be issued. Shipping Fees are non-refundable for refused packages.
